What we need from you

  • Full Insurance Name: Indicate whether it is "UK" or "Global/International" in the title.
  • Policy Number: Ideally, please provide a copy of your policy (either in the clinic or via email).
  • Pre-Authorization Code or Letter of Guarantee (LOG): This is essential for us to proceed with your treatment.

Important: If you plan to "pay and claim" later, please note that we cannot guarantee your insurance will cover the service.

How to obtain a pre-authorization code

If you are unsure how to obtain this code, follow these simple steps:

  1. Contact your insurance provider before your appointment. Use the contact details (phone or email) listed on your policy.
  2. Explain your situation. Provide a brief description of your symptoms and, if available, any diagnosis.
  3. Detail the planned services. Include the following information:
    • Consultation: Specify the doctor’s name and specialty.
    • Diagnostic Tests: List any tests you expect to undergo.
    • Treatment: Mention any known treatment plans.
  4. Specify the location. Inform your insurance provider that the services will be provided at VisitHealth.

If your insurance provider requests an estimate of the costs for the planned services, we can assist by providing this information. You can then forward it to your insurance provider as needed.
